AFAC application - Application de CCFEA

If you would like to apply for the role of Family Advisor, please fill out the form below. Your information will be kept confidential and will not be shared outside of the selection panel. In choosing Family Advisors, the selection panel (composed of existing AFAC members) aims to maintain a pool of advisors who represent the diversity of the families we serve. Members need to be available to attend monthly meetings (via videoconference or in person) for a minimum commitment of 2 years.

Interview

After reviewing an application, the selection panel will schedule short telephone interviews with a member of the AFAC executive and a member of the CHEO Autism Program leadership team.  The interview process provides an opportunity to deliver additional information about the role and expectations of a Family Advisor, to ask questions related to the application, and to answer any questions from the candidate. While we appreciate each candidates’ interest, submission of an application and granting of an interview does not guarantee selection.

 

Orientation & Trial Period

Candidates who are selected as Family Advisors will be invited to participate in an orientation and trial period. At this time a confidentiality agreement is signed and 3 consecutive AFAC meetings are attended; this is an opportunity to test out the Family Advisor role prior to formally becoming a member of the AFAC.

Si vous souhaitez présenter votre candidature comme conseiller familial, veuillez répondre aux questions sur les pages qui suivent. Vos renseignements restent confidentiels et ne seront pas partagés hors du comité de sélection. Lors de la sélection des conseillers, le comité de sélection (composé de membres actuels du CCF) vise à assurerun groupe de conseillers qui représente la diversité des familles que nous servons. Les membres doivent être disponibles pour assister aux réunions mensuelles (par vidéoconférence ou en personne) pour période minimum de deux ans.

Entrevue

Après avoir révisé une demande, le comité de sélection organise une courte entrevue téléphonique avec un membre de la direction du CCF et un membre de l’équipe de direction du programme d’autisme de CHEO. Le processus d’entrevue permet de fournir des renseignements supplémentaires sur le rôle ainsi que les attentes d’un conseiller familial, de poser des questions liées au formulaire de candidature et de répondre aux questions que pourrait avoir le candidat. Nous tenons à remercier tous les candidats ayant posé leur candidature. La soumission d’un formulaire de candidature ou la convocation à une entrevue ne constituent pas une garantie de sélection.

 

Orientation et période d’essai

Les personnes sélectionnées pour faire partie du Comité consultatif des familles seront invitées à une séance d’orientation et une période d’essai. Durant cette période, le candidat devra signer une entente de confidentialité et participer à trois réunions consécutives du CCF. Ceci permet au candidat d’évaluer le rôle de conseiller avant dedevenir officiellement membre du CCF.
